Medical Clearance
To be eligible to participate on the first official practice day for traditional/non-traditional season OR be treated by the athletic trainers, student-athletes must complete the medical paperwork requirement. Pick from the four designations below to see what fits you best then follow the instructions to complete your medical paperwork. Please contact sportsmed@hartnell.edu for any questions.
New Athletes
"I have never participated in athletics at Hartnell."
What do I need to complete?
- Physical
Print or pick up the Hartnell Physical form and take it with you to your appointment. This must be signed by an MD, DO, PA, or NP. Physicals signed by a chiropractor WILL NOT BE ACCEPTED. - SportsWare

Returning Athletes

What do I need to complete?
- Sophomore Athletic Health Questionnaire
This can be completed by visiting the Hartnell Athletic Trainers at the Main/H Clinic. - SportsWare Update
Visit the SportsWare website and update any information that has changed. You will need your email and password.
Mid-Year Transfers
"I participated at another college/university and am transferring to Hartnell in the middle of the school year."
What do I need to complete?
- Physical
- Option 1: You can request the physical from the school you were previously enrolled at and turn that in.***
- Option 2: You can get a new physical from a physician, physician's assistant, or nurse practitioner of your choosing. Make sure it gets stamped by the health care provider. Physicals from a chiropractor are not accepted. Print a Hartnell Physical Form or pick one up from the athletic trainers and bring it to your appointment if you choose this option.
***If you turn in a physical from the previous school you were enrolled in, you will need to complete a Hartnell Athletic Health Questionnaire as well. Please contact a Hartnell athletic trainer to have this done.
